While our hospitalists are employed by our private practice hospitalist group, they serve integral roles that support the facility's operational and clinical goals. They are responsible for inpatient rounds, medical and surgical admissions and discharges, surgical co-management, ICU care with intensivists, supervision of mid-level providers, and management of patients.

Timothy M. Capstack, MD, FACP, SFHM

Director of Clinical Integration and Lead Hospitalist

Dr. Capstack has directed our group of over 25 physicians, nurse practitioners and physician assistants at Anne Arundel Medical Center in Annapolis since 2009. His role includes oversight of clinical quality assurance and improvement, education of students and recent graduates, and extensive interfacing with other program directors, consultants and hospital administration. Dr. Capstack earned his medical degree from Washington University School of Medicine in St. Louis, Missouri, and completed his residency training in internal medicine in 1999 at Barnes-Jewish Hospital, also in St. Louis. He is active with the Society of Hospital Medicine (SHM) as a member of SHM’s national committee on NP/PA hospitalists. He was the lead author for the 2016 study “A Comparison of Conventional and Expanded Physician Assistant Staffing Models at a Community Hospital,” published in the Journal of Clinical Outcomes Management (, which demonstrated the efficacy and safety of the Adfinitas Health (then MDICS) hospital staffing arrangement. Dr. Capstack is a Senior Fellow in Hospital Medicine and a Fellow in the American College of Physicians. He has been board-certified in internal medicine since 1999.

Chang Choi, MD, FHM

Regional Medical Director

Dr. Choi was a hospitalist at Anne Arundel Medical Center for several years before joining Adfinitas Health in 2008 as the director of the hospitalist program at Calvert Memorial Hospital. Since then he has served on numerous committees at Calvert including the Medical Executive Committee, Joint Quality Improvement and Safety, MHAC, and others.  He has also served as the Medical Director of Informatics and served as the physician champion for building and implementing a new EHR system called Meditech Advanced Technology in 2016.  He now serves as the Regional Medical Director for Adfinitas Health. 

Dr. Choi grew up in Howard County, Maryland, and received his bachelor's degree in biophysics from Johns Hopkins University. He graduated from the University of Maryland School of Medicine, and completed his residency in internal medicine at the University of North Carolina Memorial Hospital in Chapel Hill. He is board-certified in internal medicine. He joined Adfinitas Health because of Doug Mitchell's visionary leadership and fluid response to the dynamics of healthcare changes. 

Steven Hamlette, MD

Regional Medical Director 

Dr. Hamlette began his medical career as a National Health Service Corps physician, working as a traditional internist, in an underserved area of Somerset County, Maryland.

In 2006, he developed a passion for Hospital Medicine and partnered with other physicians to form a private hospital medicine group on the Maryland Eastern Shore. In the summer of 2007, he met Drs. Mitchell and Davis and was very impressed with their vision and plans for Adfinitas Health. Months later, in early 2008, he joined Adfinitas Health.

Dr. Hamlette is originally from New Jersey, and moved to Washington, D.C. to attend Howard University.  At the university, he earned a bachelor’s degree in Chemistry. He remained at Howard University to continue his studies at the College of Medicine.  After graduation, he relocated to Maryland to accept a residency position in Internal Medicine at The Johns Hopkins University/Sinai Hospital training program. As an Attending physician he returned to Johns Hopkins University and completed the Business of Medicine Graduate Program at the Carey Business School.

He is Board Certified in Hospital Medicine by the American Board of Internal Medicine and the National Board of Physicians and Surgeons. Currently, he is one of the Regional Medical Directors for the Adfinitas Health acute care facilities. His region includes Bon Secours Hospital, Union Hospital of Cecil County, Fort Washington Medical Center, and Southern Maryland Hospital.

Ivelisse Michel, MD

Regional Medical Director 

Dr. Michel joined Adfinitas Health in August, 2009, after completing her family medicine residency at Bayfront Medical Center in St. Petersburg, Florida, where she was chief resident. She graduated from the Medical College of Wisconsin. As the Hospitalist Director for University of Maryland Charles Regional Medical Center (UM CRMC), she has been successful implementing an effective transitions of care program to reduce readmissions. Dr. Michel is also the Regional Medical Director for our Southern Region, which includes UM CRMC, MedStar Southern Maryland Hospital Center and Fort Washington Medical Center.

Managing Executive Members

Adfinitas Health employs over 325 providers. Approximately 30 percent are advanced practice providers (Certified Registered Nurse Practitioners and Certified Physician Assistants). Our advanced practice providers work in conjunction with our Hospitalist Program Directors as well as the post-acute care and assisted living community Medical Directors. Adfinitas Health is committed to developing leadership opportunities for our providers. Our Managing Executive Members are advanced practice providers who have demonstrated outstanding leadership and a commitment to excellence in addition to supporting their facility’s operational, clinical and financial goals.

Hollie Hurner, PA-C

Administrative Director 

Hollie is currently the Administrative Director at Calvert Memorial Hospital.  Her role includes managing the day to day operations of the hospitalist group, serving on hospital committees and working closely with the hospital administration.  She served in the United States Army from 1992 - 2003 as a medical specialist and attained the rank of Staff Sergeant.  Hollie received her AA degree from Chaminade University, Honolulu, Hawaii in June 2002 and received her Bachelor of Science degree from Florida Southern College, Lakeland, Florida in December 2005. She holds a master’s degree in Physician Assistant Studies from Methodist University, Fayetteville, North Carolina. Hollie is NCCPA Certified (2008 to present) and is actively serving on several committees with NCCPA. She is a member of the American Academy of Physician Assistants and the Society of Hospital Medicine. Hollie joined Adfinitas Health in March 2009 as a Physician Assistant at Calvert Memorial Hospital. She served as Lead Physician Assistant at Calvert and has filled staffing needs at several of our other hospitals prior to assuming her current role.
