Adfinitas Health stands out in its ability to utilize high-function APPs, starting with the intensive APP training program that combines didactic and on-the-job training over an approximately six-month period. The APP “onboarder” is mentored, tested, and paired with a trained and dedicated Clinical Instructor to ensure that they can successively complete the training program and function as an integrated member of the clinical team seeing up to 15 patients per day.

Regional Medical Director for Post-Acute Services in Anne Arundel County


Dr. Reshma Modi is a Physician and Regional Medical Director for Post-Acute Services in Anne Arundel County. Dr. Modi was nominated by Yesha Patel. Yesha shared, “Dr. Modi is one of the best Attendings I have ever worked with. She is always responsive, easy to talk to and a caring provider. She is brilliant with patient management and over the years has become part of my family. Thank you for working with me, Dr. Modi.”



APP and Regional Supervisor of Pennsylvania Post-Acute Services


Courtenay is an APP and Regional Supervisor of Pennsylvania Post-Acute Services. She was nominated by Susan Diana after receiving a glowing recommendation from the Admissions Director, Emily Taylor, at The Gardens at Wyoming Valley. Emily wrote, “As the Admissions Director, I have had the opportunity to see firsthand her daily dedication to her patients’ physical and emotional needs, always going above and beyond to uncover new therapies to benefit each unique case.

Courtenay’s daily visits to our facility have allowed our patients to benefit from a continuum of care and a reduction in hospital readmissions, eliminating unnecessary exposure to infection and delay in care. She communicates with each family and builds confidence from her first interactions. She is respected among her colleagues and is an important member of our team at The Gardens, as she plays a vital role in developing and executing successful discharge plans. As I meet each resident throughout their stay, it is not uncommon for me to hear accolades about Courtenay’s dedication and tenacity as she treats, comforts and advocates for each of her patients. She is the epidemy of a team player and a wonderful example of what healthcare professionals should be, courageously and tirelessly caring for her patients and producing measurable positive outcomes.”



Acute APP and CDE, Clinical Educator & Director of Quality at UPHS-M


Jill Thomas is an Acute APP and CDE, Clinical Educator & Director of Quality at UPHS-M. She was nominated by Dr. Robert Carroll and Jonathan Bjork. Dr. Carroll stated, “Jill has been coming in most weekends, without being asked, volunteering during a time of high volumes at our site. She continues to not only do her clinical work despite this, but also focusing our team on our quality goals as well.” Jonathan added, “When census has been extremely high and our morale dropping, Jill has consistently put herself out there for the good of the rest, including giving up her weekends to serve as our much needed back-up. For this alone she should be nominated, in addition to her great work with patient care and helping the team/site as she does.”



Assistant Director of Critical Care Operations and Lead Critical Care APP at Mercy Medical Center


Deb Bohan is the Assistant Director of Critical Care Operations and Lead Critical Care APP at Mercy Medical Center. She was nominated by Dr. Chang Choi.

Dr. Choi glowingly shared, “Deb Bohan embodies self-sacrifice for the good of humanity. When speaking with Deb, she has patience and imbues a sense of empathy and compassion that I can only imagine is magnified when she is speaking with critically ill patients or family members. Most recently, she scheduled herself to work 14 critical care nights in a row so that her colleague could have some time off in the midst of the 4th COVID surge without any complaints and humbly owned the situation without bragging or fanfare. Deb has completed multiple missions across third world countries and even cashed out her retirement account to help a good cause. On her spare time, she rescues dogs from across the globe and has at least one parrot that we can hear on our conference calls. =)
