As post-acute facilities continue to face the continually changing state and federal regulations, Adfinitas Health is one of the few hospitalist groups to provide experience in post-acute nursing and extended-care facilities. We understand the value of partnerships across the care continuum to improve outcomes, deliver a higher level of care coordination and reduce readmission rates.
Highly skilled Adfinitas Health physicians and advanced practice providers (APPs) in our Post-Acute Services team care for patients in a variety of post-acute care settings.

Upon training completion, oversight of the trainee’s clinical activities will continue. Each APP is paired with a physician and his/her rounding list is reviewed twice a day, allowing the new trainee to continue to learn and ask questions. Additionally, the physician will be primarily responsible for any patient that has seen an APP two days in a row (three-day rule), and patients admitted by APPs are all seen the next day by a physician.
- Regularly benchmark clinical staff member performance for quality and performance
- Recruit, onboard, and train physician assistants to work at the top of their credentials
- Provide 24/7 after-hours call teams comprised of advanced practice providers
